Legal Guardian Agreement to No Allegiance 2009 Game Day Waiver (For 15 years of age and under players)

I / WE A)___________________________ and B)____________________________as undersigning parent(s) and or legal

guardian(s) of _______________________, agree that I / WE are of sound mind and body, that I / WE are fully aware of the details of this document and the risks involved with MY/ OUR child playing this sport. I/ WE understand, I/ WE are also bound by any and all statements made within the “NO ALLEGIANCE 2009 WAIVER”, “PROPERTY RULES”, and “2009 MEDICAL RELEASE WAIVER BAD KARMA AIRSOFT FIELD FOR NO ALLEGIANCE PLAYDATE” documents. I/WE as the parent(s) and or legal guardian(s) agree, I/WE are not required to remain on site while My/Our child is playing as long as the child is of the age of 16 years or older, but if I/WE do choose to leave, I/WE knowingly give up the right of adult supervision of my child on the premises. I/WE knowingly understand and agree that there is NO SPECIFIED ADULT SUPERVISION ON THE FIELD AT ANY TIME. This means that even though adults may be present during the game day, I/WE knowingly except they are NOT there in any official monitoring capacity and I/WE cannot hold them accountable in such a capacity. I/WE understand that if My/Our participating player, and above named, is 16 years of age or under, I/WE as undersigning parent(s) and or legal guardian(s) of participating player, and above named, must be present throughout the whole game day:
